Report (For Personnel Who Will Undergo PCR Test)

With the circular dated 03.09.2021 from the Republic of Turkey Ministry of Labor and Social Security, a weekly PCR test obligation has been imposed on personnel who have not been vaccinated.

Within this scope, I accept and undertake that I will undergo a PCR test once a week and deliver the test result to the workplace physician.
